Field Trip Terms and Conditions

Destination

  1. Age group 4 – 11 years old: Museum of Science and Industry Chicago.
  2. Age group 12 or older: 360 Chicago - Observatory on the 94th floor of the John Hancock Building with a tilting glass lookout.

Requirements

  • A field trip participant must be 4 years old and older.
  • A field trip participant has to be registered as a Muktamar participant.
  • Participants who are 11 years old or younger can only choose the destination: Museum of Science and Industry.
  • Participants who are 12 years old or older can choose either going to Museum of Science and Industry or to 360 Chicago (John Hancock Building)

Cost

  • The cost of this field trip is US$ 30 per person including transportation.
  • Note: Volunteers will not be charged however need to register at the registration desk.

Departure/Arrival and Field Trip Procedure

  • The field trip will be on Thursday, December 26th, 2019.
  • Only those who are already registered for the Field Trip can join this field trip
  • All participants will gather in a designated place (to be determined prior to the field trip date) in the Muktamar location no later than 9:15 AM.
  • Bus will depart the Muktamar location by 10:00 AM
  • Each participant is responsible to be at the gathering place on time to avoid missing the bus.
  • The field trip committee is not responsible for any participant who is missing their bus due to late arrival at the meeting point.
  • Participants will be assigned into groups, and each group consists of 10 participants plus a group leader.
  • Group leader will have names of all participants in his/her group
  • Each participant needs to know who is his/her group leader and has to follow direction from the leader of his/her group
  • Participants should wear their Muktamar name tags all the time during this field trip
  • Participants should follow rules and regulations set by the field trip destination hosts, bus drivers, and Muktamar field trip committee
  • Participants should communicate any concern during the field trip to his/her group leader, and the group leader will communicate that concern to one of the field trip committee members
  • The duration of this field trip is approximately 5 hours
  • Participants will arrive back at the hotel around 2:30 PM
  • Parents or designated trusted persons are expected to welcome their kids at the same designated place above when they arrive back at the hotel.

Cancellation

  • The cancellation is possible at any time by sending an email to muktamar@imsa.us.
  • The payment can be transferable to other participants. The payment can be reimbursed with the following rules (note: this rule will be strictly enforced):
    • Before or on Dec 10, 2019 at 11:59 PM PT: 100% refund.
    • Before or on Dec 23, 2019: 50% refund.
    • After Dec 23, 2019: no refund.

Risks and Waiver of Liability

  • Participant's guardian acknowledges and accepts that the mode of transportation and the nature of the field trip/activity may expose the participants to hazards or risks that may result in illness, personal injury or death.
  • The guardian recognizes and acknowledges that certain risks of harm are or may be inherent in the various activities contemplated herein and that the committee cannot control all of these risks.
  • The Guardian acknowledges there may be physically strenuous activities and certify by signature that the participant is physically fit and able to participate.
  • The guardian agrees that while participating in the field trip or activity, the participant will abide by the guidelines set forth in the committee Code of Conduct and agree to adhere to all applicable rules, regulations, and laws while participating in the field trip or activity.
  • The Guardian understands that the committee is not responsible for any medical expenses associated with any property or personal injury the participant may sustain.
  • The guardian and participants should make their own arrangements with respect to personal insurance, along with travel insurance.

General Conditions

  • IMSA and MISG reserve the right to amend any part of the field trip program if necessary, at any time.

  • We reserve the right to alter these Terms and Conditions at any time. If the alterations constitute a material change to the Terms and Conditions, we may notify you via internet mail. What constitutes a material change will be determined at our sole discretion, in good faith and using common sense and reasonable judgment.

Participant hereby agrees to abide by the terms and conditions as provided above and confirm that the information given in the registration form is both true and correct.
